Both applications are developed and published on Google Play by Spark Media (the “Developer”) on behalf of Inova Health System (the “Owner”). Inova Health System owns and operates the Services and is responsible for the handling of any information described in this policy. Spark Media acts solely as the developer and publisher of record.
These applications are provided free of charge and are intended for use as is. In this policy, “we,” “us,” and “our” refer to Inova Health System, and “Service” or “Services” refers to the PFDR and MFW applications.
This page informs users regarding our policies on the collection, use, and disclosure of personal information in connection with these Services. If you choose to use our Services, you agree to the practices described in this policy. We will not use or share your information with anyone except as described here.
Accounts and Personal Information
Our Services do not offer account registration and do not ask you to create an account. Users sign in using credentials that have already been issued to them by Inova Health System. We do not collect names, email addresses, or other personally identifiable information through the Services for the purpose of creating or maintaining a user profile.
Log Data
Whenever you use our Services, in the case of an error in the app, we may collect data and information (through third-party products) on your device, referred to as Log Data. This Log Data may include your device Internet Protocol (“IP”) address, device name, operating system version, the configuration of the app at the time of the error, the time and date of your use of the Service, and other statistics. This information is used solely to diagnose and fix technical problems.
Cookies
Cookies are files with a small amount of data that are commonly used as anonymous unique identifiers. They are sent to your browser by the websites you visit and stored on your device’s internal memory.
